family law - custody trial- proposed judgment

The judge asked for both parties to write a proposal judgment with a rationale. i was wondering if there was a certain setup and how exactly it should be completed?

Your best way forward is to get an attorney to do this for you.

However, the general format is to state a finding of facts based on the testimony and then based on the facts and the law to write a justification for the judgment.
