what form would I use to compel an employer to furnish me with accurate information concerning someone's wages?

am presently involved as a defendant in a family probate modification motion and am Pro Se. Would I use a subpoena or a summons?

we are in the discovery phase. can this be included by mention?

can I request copies of recent income tax returns from the other party and if so what form would I need and how would I word it? How many days before trial date could I do that?

You need to subpoena the keeper of the records for the employer and state what information you require them to present.
