Legal Question in Family Law in Massachusetts

Help on Form

On the Mass form, #5 states "On or about __________________. 19______, the defendent_______________________________________________________________________________________________________________________________________________________________________________________________________________________" What am I supposed to fill in here?

The next #6 continues "Wherefore, plaintiff requests that the court:" with a list of choices for granting a divorce, grant custody, etc.

Please help. Thank You

You should insert the factual basis for the grounds for divorce

Your spouse is the defendant, so put the name of

your spouse after the word "defendant."

Because you are bringing the divorce action, you

are the plaintiff. The form requires you to

identify the grounds for the divorce. Read the

list and check off those grounds which you

believe apply to your marriage.
