the mother of my child is moving out of the state

The mother of my child recieved primary physical custody of my son 10 months ago, at which point she moved within the state to ''improve her quality of life'' and become a manager of a pizza chain. To date she still does not have a job nor does she even have a bed for my son to sleep in, and now she is planning on moving out of the state. I will not agree to the plan that she has for visitation, and she has moved 7 times, my son has already been to 6 different schools and he is 8 years old. She should have never received custody the last time we were in court how can I prevent her from moving again and taking my son?
