what is a written proposed judgement with a rationale? is there a certain format?

The proposed judgment is what you are asking the court to do. You should number each point that you want the Judge to agree on. The caption (top part of any court document) should always be the same. The rationale is the reason as to why the Judge should rule based on what you are asking for. You should contact an attorney for more specific details.
