You don't technically have to hire a lawyer, but it is strongly suggested. Our office, and likely others, would charge a reasonable flat fee to get this done for you. There is no form, per se, you would need to prepare the an actual qualified domestic relations order, or QDRO (which is a very complicated document), and then prepare and file a motion with the probate and family court allowing the QDRO. A QDRO form should be prepared by someone familiar with it to avoid any adverse tax consequences. However you do not need to hire an attorney to help you prepare it. As Mr. Vaughn-Martel stated most experienced attorneys would help you prepare the form and a motion for the court to allow the QDRO.
