You do not need an attorney in order to change your name. You can find the required forms at your local Probate and Family Court. Ask the clerk.

If you would like, most attorney would be able to assist you in filing the forms for a small flat fee.

You do not need an attorney in order to change your name. You can find the required forms at Plymouth County Probate and Family Court website.

However, an attorney would be able to assist you in filing the forms for a small flat fee, and would be advisable if there are any complex issues such as marriage names and/or criminal records issues. Good Luck and Happy Holidays.
