How long does it really take to have one's patent request accepted?
Is there a way to make sure it is accepted initially instead of the initial rejection then a yearly dance till approved?

It completely depends on how busy the patent office. In order to increase the chances that it is accepted initially, one would have to write extremely narrow claims, that would probably be useless because they would be very easy to design around.
