Can my supervisor provide me prescription pain killers not prescribed to her and after a personal conflict that I chose to remove myself from, provide management with a bad performance report for me? Should I file a complaint to disclose and protect myself from retaliation on my two week probation? Other work errors where brought up but I am almost certain she prompted the unscheduled performance evaluation by complaining about my work quality after personal conflict.

It is illegal to give any prescription medication to someone it is not prescribed.
As to the issue regarding the personal conflict without more information, it is difficult to advise you how to proceed. If you believe that the supervisor has a personal animus (extreme dislike) towards you that impacts her judgment and evaluation, you should to talk to someone in HR, assuming there is a HR Dept. or person.
