You should review the statute, because I am writing this by memory.
A 14-day notice to quit should include names of all tenants, their unit address, monthly rent rate, amount of rent unpaid to date, name of landlord, landlord's business address, and proper language indicating that the notice is given due to non-payment of rent. It shold specify whether the rental agreement is a lease or a tenancy at will (any "unwritten" tenancy is a tenancy at will). If the tenancy is a tenancy at will, and the tenant has NOT had a notice to quit served in the prior one year, the tenant must be granted ten days from delivery to pay the full rent due and owing, allowing them to "cure" the non-payment. It should be signed by the landlord, and have proof of delivery ("service").
