Providing Proof in Order to Break a lease

I've lived in my current apartment for almost five months now. Everything was fine until the end of November, when it got really cold out and we didn't get very much heat. We've complained a few times via phone, and all we got was duct tape along the sides of the windows. We've recently also had a mice problem, which the maintenance guy took care of by handing me some glue traps but never checked for holes like he said he would. How do I quickly prove that they're not keeping their end of the deal and break my lease without owning them money? I want to move out as soon as possible, and have in fact found a new place, but feel like there's not enough time to provide a paper trail for the issues. Thanks in advance!

Request the Board of Health conduct a formal inspection. And give notice as required by the lease that you are terminating due to constructive eviction.
