need legal forms for rto

we are currently renting to own a property in which we plan to sublet out rent to own also we need to know the proper forms to do this in massachusetts

There are no "proper" forms. There must be an agreement in writing that reflect completely and in detail the understanding between you and the deed holder. Moreover, once this agreement is signed and I would have the signatures notarized, I would suggest filing it with the registry of deeds.
