I need to request a hearing by a judge regarding my daughter's entitlement to benefits. When I fill out the document should I request to appear and should I have an attorney present? Thank you.

You should request to appear, but you may be better off having an attorney file an appearance to deal with the regulatory and evidentiary issues.
