Legal Question in Tax Law in Massachusetts

Hi, I have a question regarding tax law. I tutored for a company as a contract worker. I made 1440 dollars during three months I was employed, I was payed by the hour and they did not withold anything. I filed taxes jointly with my wife on time, and this company for whom I did not work for in more than a year is asking me to fill a 4669 IRS form, do I need to and what is the risk? Thank you

Form 4469 is acknowledgement that you received the $1,440 from the employer for that year and included the income in your tax return.

If the IRS asks you to fill it out you are required to do so. If you included the income in your tax return for the year in question, you do not have a problem. I assume you never got a 1099 from the company you worked for, so this is there way of proving you were paid and you declared the income. If you did get a 1099 then indicate that on the form and attach a copy of the 1099 to it.
