Unfortunately, telling you how to fill out the form will open me up to liability if something went wrong. But you do have options:

You should know that the personal administrator is entitled to have attorney's fees paid for out of the funds of the estate, so if there are any assets in the estate at all, you shouldn't have to spend your own money.

If you still don't want to hire an attorney, you can contact the Public Administrator for the county where the decedent died and/or held property. The PA is appointed by the governor to handle estates where there are no heirs in MA or where the heirs do not desire to handle the estate probate.

I would love to help you in this. If you feel comfortable hiring and attorney, payable out of the estate, please give me a call. If your relative died in or had property in Suffolk County, the partner at my firm is Public Administrator. Otherwise, feel free to contact the PA in your county.

Let me know if I can be of help.
