Legal Question in Workers Comp in Massachusetts

I had an injury at work about 3 years ago and at first it seemed minor and that it would go away by itself so I did not seek treatment right away. I also did not file a worker's compensation claim. The injury did not get better (it got worse) and tthree months ago I had surgery and used my medical insurance to cover it. My insurance carrier just sent me an "abrogation" letter asking me to fill out a questinaire on whether the injury happened at work and whether I filed a workers compensation claim. Is it too late to file a workers compensation claim with my workplace (I still work there) now and if it is too late, can my medical insurance carrier deny reimbursement because I should have claimed workers compensation when the injury happened?
