What kind of form do I need to do an ''on-the-street'' interview for our church? We are a non-profit video ministry and need to know what we need in order to video tape people in a public setting.

You really need to consult an attorney with intellectual property experience. The answers here vary depending on what use you will make of the tapes, whether the tapes will be sold, whether they will be broadcast, and whether any person might take offense at the way they are portrayed on the tapes.

I would advise you to consult an expert in this area and rely on his or her opinion.
