Legal Question in Civil Rights Law in Michigan

I filed a case with the EEOC for disability and age discrimination. I was given the right to sue in US District Court.

I filled out the civil cover sheet with both 442 and 445 which said:

442 Employment

445 Amer w/Disabilities-Employment

At the clerk's office they had me circle just one so I circled 442. I wanted to fill out a second form but they said no the judge would see everything.

Did they trick me? Will my disability evidence be thrown out on some technicality? Or am I still valide for both disability under the Americans withdisability act and age discrimination this way?

Thank you in advance for any help you can render. I am worried about being tricked because fraud, perjury, and tampering with evidence already occured at the EEOC hearing.
