Legal Question in Credit and Debt Law in Michigan

Being sued for a credit card I have never applied for.

what form do I need and where do I get it? for: need proof of ownership, proof of purchase, proof of signature, and proof of purchase history. Is it Discovery of Doc. form? Where can this be found

There is not a preprinted form for discovery. Attorneys draft the questions and requests for documents themselves and they are specific for each case/issue.

If a lawsuit has already been commenced against you, I suggest contacting a lawyer immediately to answer the complaint and talk to the creditor. If you don't answer timely, they could get a judgment against you, which will make things more difficult. Obtaining documents/discovery from the creditor is simply done by a written request, but there is a specific format you should follow.
