does this normally get reduced to a misdemeanor for someone with no prior arrests, upstanding citizen, married, employed as a health care profession

Not necessarily. In what county is the case filed? That is the biggest consideration. Also, to what judge is the case assigned.

Is the defendant a licensed health care professional? If so, a conviction will have serious consequences.

Some police officers tend to overcharge public urination as gross indecency. If that's the case with you, the answer is probably yes.

But don't fool with this yourself. If you can't afford an attorney, get a court appointed lawyer. If convicted, a future charge of public indecency wil require registration as a sex offender. The present charge may get you fired from your health care job.
