Re: Name Change after divorce to maiden name

The maiden name can be restored within the Judgment of Divorce. The attorney drafting the judgment should be asked to include the restoration of the maiden name. After the judgment is entered your mother can go to the Secretary of State's Office nearest her home and request that her voter's registration and driver's license be corrected. She can also go to the Social Security Office and have them correct her social security records as well. All of her creditors should be provided with the name change as well.

Good luck.
