I am trying to find out what forms i have to fill out and turn it to motion the judge to recognize me as co-counsel in my parental right procedings.

If you are a party, you do not need to motion the court to be co-counsel. You need to just show up at the next court proceeding, or you can call the court to find out what you need to do. The court cannot give you legal advice, but there may be some procedural requirements it can help you out with. It sounds like you need some legal counsel of your own, because your question raises some questions of my own. There is no such form as the one you are requesting. Attorneys actually write their own motions from scratch.
