Legal Question in Family Law in Michigan

Name discepancy

Recently realized my birth certificate lists my first name as--name removed== rather than--name removed-- All other legal records since my birth (baptismal,marriage, draft card, drivers license and professional licenses) list--name removed--as my official name. I would like to obtain a passport but am unable to submit confirmation that--name removed--and Henry are the same person. I intend to continue to utilize the name--name removed-- Advice on process for obtaining passport is appreciated.
