filing for divorce on my own

I purchased a divorce packet from the local courthouse. In the packet are questions pertaining to children in my marriage. Does a person have to fill in info on a child from the marriage if a parent has voluntarily terminated their parental rights to the child? Can the papers be filled out as no children in common. The termination of parental rights is legal and signed by a judge. Can a party being served divorce papers be served by mail in prison?
