termination of parental rights
My sons father has been in and out of prison since our childs birth. i have had no contact with him for a year.I would like him to give up his parental rights to our son. Is there a form i can get that has to be signed by him that will give up his rights as a father? i understand he would have to agree to this but i have no idea where to find such paperwork. please help.

You should do this through an adoption lawyer. A simple renunciation of parental rights may not be accepted by a court. Aside from custody and parenting time, the effect of the renunciation might operate to cutoff child support. A court probably would find that not in the child's best interest.
