What form or publication do I need if I want to pursue a claim of money owed, over $5000

Contrary to popular belief, attorneys actually draft their own complaints and their own documents (sometimes we borrow and once in a while there are court-approved forms we can use). The practice of law is not about finding the right form to fill out. If that was the case, we would not have to go to law school.

You cannot file a claim in small claims for $5000, the limit is $3000.00. You can file in District Court up to $25k. While you could represent yourself in the suit and file your own summons and complaint, it may be worth it to hire an attorney who may be able to negotiate a settlement that would include attorney fees and court costs.
