where do i find info on fileing a malpractice lawsuit

i need to file a lawsuit for 1 million dollers against a doctor who harmed me i need to know which law book to look in to be able to complete the requirements to submit this info to a court

To even ask such a question indicates that you have a major misunderstanding of what is involved in filing a malpractice case. I really do not mean to insult you, but the degree of difficulty in you handling your own malpractice case would be equivalent to a kindergartner rebuilding a Ferrari engine.
