Adding a name to a house title

My mother wants to add my name to her house as co-owner. We live in Michigan (Kalamazoo area). It has never held anyone else's name but hers. How would we go about doing this? Who would we need to contact?

She would simply execute a Quit Claim deed granting the property from herself to herself and you, joint with rights of survivorship.
