Advance Directives

It is a statement of your health care choices in the event of a serious illness or injury. It is your voice when you cannon speak for yourself.

It can say which family members or friends you would want ot make decisions for you. Advance directives should be in writing as proof of your wishes.

A living will and a patient Advocate Designation or Durable Power of Attorney for Healthcare are examples of advance directives.

All of these directives can give instructions to others about the type of medical care you would or would not want.

The Patient Advocate Designation for Healthcare lets you name an advocate. An advocate is someone who would speak on your behalf. An advocate would make sure to carry out your wishes.

In the state of Michigan, the Patient Advocate Designation for Healthcare is the best type of advance directive to write.
