Simply go to the court in which your case will be heard (or for counties with multiple courthouses, such as Hennepin, Ramsey or Dakota, any of the county's courthouses) and request a subpoena.

You will then need to serve the subpoena upon the desired witness. This can be done by obtaining an Admission of Service from the witness, and then filing this document with the court together with the original subpoena.

Another way to serve a subpoena is by having someone 18 or older who has no personal connection to the case go and personally deliver the subpoena. Many law firms will engage process servers who charge a fee based on distance and how many attempts they must make. There are numerous companies who do nothing but serve legal process such as subpoenas. The person who serves the subpoena will need to complete an Affidavit of Service which you will file with the court along with the original subpoena.

Good luck!
