Legal Question in Family Law in Minnesota

In 2008 a judge sent out the Findings of fact conclusions of law and order. Within that document I was advised by the judge to schedule a review hearing in 2009 - My request was granted - I am unable to afford an attorney this time and will be representing myself. I must file an affidavit 7 days prior to the court date detailing income and current costs for medical/dental insurance - where can a find an affidavit form that I myself can fill out?

All that is required for an affidavit is that it is a notarized statement, You can type up your narrative as to finances and attach any relevant documents as exhibits.
