Birth Certificate

My 14 year old son's Birth Cert. has my maiden name on it even thou my husband was there at the birth and we thought he filled out correct forms we were married 12 months after the birth and my son has used my married name but how do i go about getting this changed. i am not getting any where with the Minnesota dept of health. please help me in the right direction thank you

If you are living in Hennepin County, you can acquire a form for a change of

name ant the Hennepin County Court Administrator's Office. It will be a

simple process so long as both parents agree on the name change.

The Hennepin County Court Administrator's Office is on the 2nd floor

directly across from the Department of Vital Statistics in the Government

Center 300 South Sixth Street. Hennepin County also offers assistance in

filling out the forms.

Ramsey county also has forms and a pro se clinic for help.

A name change proceeding in District Court would be your best bet. They are easy to do and most people do it without an attorney. In most Minnesota counties, the clerk of court can provide forms and some instructions.
