Divorce - Name Change

I recently divorced (we did not go through a lawyer) and in the divorce decree indicated that I would change back to my maiden name. If I now want to keep my married name, what is the process to do so? Is there a form that I could complete and file that would amend that part of the divorce decree?

You would have to file for a name change. It is an administrative process and would require no hearing.
