DO I FILL OUT THE AFFIDAVIT STATING NATURE OF THE ACTION FORM? THE AFFIDAVIT FOR PROCEEDING IN FORMA PAUPERIS I CHECKED 3.a-THEN GET NOTARY PUBLIC WHEN I SIGN.-THE ORDER DENYING/GRANTING IN FORMA PAUPERIS I FILLED THE CASE HEADING ONLY. DOES THAT SOUND RIGHT?? THANK YOU
