In an uncontested custody change (from joint legal/ joint physical to sole custody) what forms need to be filed, and does there need to be a hearing or is there a specific form or process for surrendering custody?

The answer depends on your county. In amny, you may file a Stipulation and Order to memorialize the agreement. In such a case, a hearing is not necessary. However, there are no forms for that purpose and it would have to be drafted by counsel.
