How do I update the deed?

We have a cabin, my great-greatgrandpa's name is on the deed, he died long time ago and my grandpa lives there and he just pasted away now my Dad and I want to update the deed and put it in my or his name so I can pass it down properly to other generations, What steps do we have to do??? I hear its going to cost alot is that true??
