general affidavit under oat

I witnessed a incident at work and the person involve want me to sign a general affidavit under oat form. Do I have to sign this form in the present of a judge or notary public for it to be legal and binding in court? to clarify my question do I have to appear in court to testify?

Generally, the affidavit is used to substantiate the facts to make it easier to settle lawsuits. However, if the case goes to litigation, then you would have to testify in person.
