I won a small claims judgement 2,000.00 some dollars, almost ten years ago. What form do I file in the court to 'revive" or keep a judgement active after the ten year period for collections?

It sounds like you need to file a Renewal Affidavit. You could probably get an attorney to draft one for you for less than $100. Don't hesitate, as you could lose your ability to collect your judgment if you fail to renew in time. It may already be too late. Let me know if you need a referal.
