I am required to petition my county court to receive permission for an LDP (limited driving privilege) as a result of losing my license for a DWI. I just need a basic form to fill with my info and to address the court. I cannot afford a lawyer because i'm a widowed single mom.

There may be generic forms available on the internet, but they are likely not proper. You may ask the clerks of the circuit court of the county whre you live, to see if they have forms. You will problaby more more likely to succeed in yrou attmetp if you have legla counsel either draft teh pleading or assit you in drafting them. You don't know how little it costs until you try to get an attorney to do it for you. Many attorneys offer a free or low cost intial consultation. You shoud ac tfast, as you may hve a very limited time in which to make your request for a temporary resticted license.
