My father is mentally and physically incapable of taking care of himself. If he can't/won't sign the power of attorney, what is my next step?

You may have to apply to be appointed his Guardain and Conservator with the Probate of the County in which your father lives. If his doctor agrees, then the county may assist you in pursuing this. If not, then you will likely have to hire private counsel to investigate your options.
