Do you need permission to use a song in a not-for-profit work out video; that will not be sold for gain?

If the video being is being sold, then you need to get permission or license to use a copyrighted song or image, or you may subject to successful litigation from the copyright holder(s).

Yes, legally speaking, you do. Not-for-profits are not exempt from copyright infringement. Permission is generally not hard to get if the sales will not affect the owner of the copyrighted song, and royalties are generally insignificant in amount unless you are planning a huge distribution of this video.
