Are their forms avalabe when the respondent has left the state?

You should ask the clerk. Attorneys usually practice law rather than provide people with forms.

Also, I would advise you to use an attorney to perform legal services rather than attempt to represent yourself. Most people who represent themselves in legal matters wind up with problems that could have been avoided if they had used an attorney. You do have an absolute right to represent yourself, but it may not be the best thing to do.
