limited non durable power of attorney for child care

Does this written document have to be signed by a notary or may it be signed by 2 witnesses as well as the parties involved?

The attorney preparing the legal document will advise you of the signature requirements. Or, if you are doing this yourself, I would suggest that you read the pertinent statutes.
