single surrogate mother

In the State of Missouri when an unmarried woman has a baby, can the biological father's name be placed on the birth certificate both to show who the father is and for the child to have the father's last name?

Yes. Although I am only licensed in IL, I could not possibly imagine any state not allowing the biological father to be on the birth certificate. As to the name, almost any word in the dictionary can be used as the last name

Yes, but therre are many other legal consequences of having a child out of wedlock. The biological father will have no rights to see the child, and the mother will have no right to collect child support from the father without some legal action being taken. I would suggest that you consult with an attorney to discuss the various legal aspects of this situation. You should consider that the child has legal rights too, and no will look out for them unless you do so by consulting with an attorney and taking the appropriate action.
