How do we remove a name and add a name from a property loan?

The appears to be at least two options for the situation you described. First, the person who wants to be on the loan documents can purchase the property. Or, that person and all one want to stay on the loan refinance through the same lender. This may require a quit claim deed from the one wanting off of the loan, and approval of the lender.
Secondly, all the people connected with the loan can ask the lender to agree to substitute the new person for the one that wants off. This would basically be a loan modification. The lender will likely want to check the credit worthiness of the person to be added, and require a deed or disclaimer from the person wanting off of the loan.
