need forms to file for financial conservatorship of my sons (12 yrs old) finances

I'm a little confused - do you need to apply to be appointed as a convervator, or do you need to file a settlement for an existing conservatorship? Regardless, you should be able to get the forms at the Probate Court in the county of residence. Unless you have experience in this matter you will need the assistance of an attorney in filling out the settlement. They can be confusing. If you are needing to apply to be appointed as a conservator, then you definitely should consult with an attorney concerning the procedures and requirements.
