Rules with Witnesses

Can the attorney who is head of the trust can name himself as every position...i.e. executor, trustee, etc.? Can said attorney use himself and/or his family members as witnesses and official notary when one is an attorney and also a public notary?

I am sorry, but your question does not include enough of the facts of your situation for an attorney to provide a response of much value. Nothing prevents an attorney who is also a Notary from notarizing documents. Family members are often used to witness signatures since they are the ones usually present. If you would try again with a more complete statement, I believe that you might get a better response.
