Name change

I want to change my last name. I am divorced and currently using the name from that marriage. I don't want to change my last name to my maiden name but to a different last name. I would like to know how to go about this.

You must file a Petition to Name Change, with supporting documents, in the Supreme Court in order to legally change your name. It is a fairly uncomplicated proceeding.

If you are intent on doing it yourself rather than retaining an attorney, you should check your State Court Web Site for a Name Change Forms package.
