i need to do this in order to transfer ownership of deceased mothers life insurance policy i am the insured how do i file the affidavit at courthouse

Best solution is to hire an attorney who does probate work. If you are planning to do this on your own, most county courthouses have a law library that will allow you to search for relevant forms and instructions on how to use them. You pay for the copies of forms you need. If your courthouse does not have a law library that permits public access, you may use the law library at Creighton or UNL law schools during regular library hours.
