If a chapter 13 has been confirmed by the court (and is completly wrong due to the negligence of the attorney) can it be changed?

Yes, if you are the Debtor, you can amend the plan and seek confirmation of the plan, if you are a creditor and it has been less than 10 days after the order of confirmation is entered, you can appeal.
